PTCE Exam Structure: 90 Questions, 110 Minutes & the 4 Domains

Scored vs. Pilot Questions

Of the 90 questions you will answer, only 80 count toward your score. The other 10 are unscored pilot questions that PTCB uses to validate new items for future exams. You will not know which questions are pilot — they are distributed randomly through the exam — so treat every question as scored. There is no penalty for guessing, so never leave a question blank.

The 4 Content Domains

The official PTCB content outline organizes the exam into 4 domains. The percentages show how much of your score each domain represents:

1. Medications — 35%. Brand and generic names, therapeutic classes, indications, dosage forms, and drug interactions, built around the top 200 drugs.
2. Patient Safety and Quality Assurance — 23.75%. Medication error prevention, high-alert medications, look-alike/sound-alike drugs, and safe dispensing practices.
3. Order Entry and Processing — 22.5%. Reading prescriptions, sig codes, day supply, refill logic, and third-party billing.
4. Federal Requirements — 18.75%. DEA controlled substance schedules, recordkeeping, HIPAA, and the federal rules of pharmacy practice.

Pharmacy Content You Will See Again and Again

Across the 4 domains, these pharmacy topics appear disproportionately often:

• Sig codes — q.d., b.i.d., t.i.d., p.r.n., p.o., a.c., and h.s. appear in order entry, day-supply math, and safety scenarios.
• Top 200 drugs — lisinopril, metformin, amoxicillin, atorvastatin, levothyroxine, omeprazole, albuterol, metoprolol, warfarin, and clopidogrel are the kind of brand/generic pairs the exam expects you to know cold.
• Pharmacy calculations — conversions (1 tsp = 5 mL), days of therapy, and concentration problems.
• DEA controlled substance schedules — schedules I through V, which drugs belong in each, and their refill rules (Schedule II prescriptions are never refillable).
• High-alert medications and interactions — insulin, warfarin, and opioids, where a small error causes serious harm.
• Order entry scenarios — what to verify, what to clarify, and when to involve the pharmacist.

How to Pace the 110 Minutes

With 90 questions in 110 minutes, you have about 73 seconds per question — just over 1.2 minutes each. That is comfortable if you keep moving. Use this rhythm:

• First pass: answer everything you know, flag the rest.
• Second pass: return to flagged questions, eliminating wrong answers first.
• Never leave blanks: there is no guessing penalty.
